HealthMedia® Care for Your Health™ is a management program for those with chronic conditions including but not limited to: asthma, high cholesterol, osteoporosis, diabetes, heart conditions and allergies.
Care for Your Health provides a personalized plan created by a team of experts including a physician, a health coach, a psychologist, and a personal trainer. You'll receive help with sometimes overlooked issues like managing common symptoms, establishing a routine for better medication compliance, strategies for coping with stress and practical tips for getting a good night’s sleep.
How to Get Started
- Access your GO-TO account or create an account if you don't have one
- In GO-TO, open your member page and click on the orange “GO-TO Healthy Choices” bubble
- Review the welcome letter and privacy policy
- Complete the Health Risk Assessment
- Choose Care for Your Health.
