Member Benefits for Quitting Tobacco
Physicians Plus is committed to helping its members (whether an adult, teenager or senior) quit using tobacco. Our Nicotine Cessation brochure describes the medications and programs available to our members.

Good Health Bonus: Members who complete an approved smoking cessation class at a participating facility are eligible for up to a $100 reimbursement. Members must pre-pay for the class, and submit the receipt upon completion.

Covered Tobacco Cessation Medication Therapy: Tobacco cessation products are now on the Physicians Plus drug formulary. This benefit applies only to those members who have prescription drug coverage, and the benefit requirements are listed below:

  • Members must obtain a prescription from their primary care physician.

  • Members are eligible to receive 3 consecutive months per calendar year of tobacco cessation therapy.

  • The pharmacy will bill the insurance company directly, and members must pay their formulary Tier-1 generic or Tier-2 brand copay.

Covered products include Bupropion SR, Buproban, Nicotine Patches (over-the-counter and prescription), Nicotrol Nasal Spray, Nicotrol Inhaler, Nicotine gum (OTC), and Commit Lozenges.

Attending tobacco cessation classes is strongly recommended, but not required for prescription coverage.

Office-Based Counseling Provider Reimbursement: Physicians Plus reimburses healthcare providers for smoking cessation counseling.

GO-TO Healthy Choices: Breathe
Breathe™ is a smoking cessation program for smokers who feel ready to quit and identifies their primary motivations for quitting, the factors favoring their success, and their barriers to quitting. Participants receive a customized action plan and follow up newsletters to stay motivated throughout the quitting process.
