For those currently enrolled or those starting a Weight Watchers program, Physicians Plus will reimburse $100 toward the annual cost of the program. Participants must meet the participation guidelines as outlined in the general Good Health Bonus program, and one of the following criteria.
There are three ways to receive reimbursement:
- Be enrolled in a traditional program and show proof of attending 10 meetings during a 12-week period.
- Be enrolled in a community program at least 12 weeks in length, and show proof of attendance at 10 out of 12 weekly meetings.
- Be enrolled in an "At Work" program at least 10 weeks in length, and show proof of attendance at 8 out of 10 weekly meetings.
Participants should obtain a copy of the Physicians Plus Weight Watchers eligibility form by either downloading it here or by calling Member Service at 608-282-8900 or 800-545-5015, and requesting one be sent by mail.
Please take this form to all meetings attended to have it signed by your facilitator to verify your participation.
