Dependent terminations cannot be processed over the phone. Please complete the Group Enrollment/Change Form and fax it to: (972) 577-7788, or mail it to:

Note: The Consolidated Omnibus Budget Reconciliation Act of 1986 (COBRA) requires a participant to notify the policyholder/plan administrator of a divorce or legal separation, the date on which a child ceases to be an eligible dependent (as outlined in the policy), or any other event that results in loss of eligibility for coverage, within 60 days of such an event.
How is a Certificate of Creditable Coverage obtained after a subscriber or dependent termination?
Physicians Plus Insurance Corporation’s enrollment department automatically generates the Certificate of Creditable Coverage, once termination of coverage has been provided. If the subscriber or dependent does not receive it within a reasonable amount of time, he or she can contact the Member Service department and request that one is sent.
